Background: Modified radical mastoidectomy (MRM) is a commonly performed procedure for chronic otitis media of the squamosal type. Postoperative mastoid cavity problems such as persistent discharge, delayed healing, and frequent cavity care remain challenging. Mastoid cavity obliteration has been proposed to improve postoperative outcomes; however, its benefits over non-obliterated cavities require further evaluation.
Materials and Methods: This observational, descriptive longitudinal study was conducted in the Department of Otorhinolaryngology–Head and Neck Surgery at a rural tertiary care hospital over a period of two years. A total of 35 patients aged 5–80 years with chronic otitis media (squamosal type), previously operated with MRM either with or without mastoid cavity obliteration, were included. Patients were followed up on postoperative day 7, and at 1, 3, and 6 months. Outcomes assessed included ear discharge, pain, giddiness, wax formation, and mastoid cavity epithelialization. Statistical analysis was performed using appropriate tests, with p < 0.05 considered significant.
Results: Mastoid cavity obliteration showed a statistically significant reduction in ear discharge and improved cavity status at 3 months compared to non-obliterated cavities. Postoperative pain and giddiness were comparable between groups. At 6 months, both groups demonstrated similar rates of dry ear and complete epithelialization.
Conclusion: Mastoid cavity obliteration provides better early postoperative outcomes without increasing complications, while long-term results are comparable to non-obliterated cavities.
