One of the multiple clinical manifestations associated with hypothyroidism is pericardial effusion. Pericardial effusions are commonly seen in cases of severe hypothyroidism, however large to massive pericardial effusions with cardiac tamponade are exceptionally rare. We report an atypical case that was diagnosed to have massive pericardial effusion and early echocardiographic features of cardiac tamponade secondary to primary hypothyroidism. Following repeated pericardiocentesis and thyroid replacement therapy, eventually the patient’s symptoms improved with no recurrence of pericardial effusion and is on regular follow-up.
