Abstract Background: The District Residency Programme (DRP) was introduced by the National Medical Commission (NMC) in India as a compulsory three-month residential training for broad specialty postgraduate students admitted from 2021 onwards in government, private medical colleges, and deemed universities. This study aims to assess the strengths, weaknesses, opportunities, and threats (SWOT) of DRP, and to evaluate residents' exposure to diverse patient cases, resource availability, interdisciplinary collaboration, and the challenges and stress they face. Methodology: This descriptive cross-sectional study incorporated a SWOT analysis to evaluate the challenges and stress faced by resident doctors during DRP postings. The study was conducted at the Department of Psychiatry, Kanti Devi Medical College, Mathura, with residents posted at district hospitals over three months. A total of 45 postgraduate residents from the 2021 and 2022 admission batches participated. A pre-tested, self-administered, semi-structured questionnaire in English was used to collect data. Results: Of the 45 participants, 23 (51.11%) were male and 22 (48.89%) were female, with a mean age of 28.54 ± 4.31 years. The majority (42.22%) were aged 26-30 years, and 29 (64.44%) were unmarried. During DRP, 23 (51.11%) participants reported exposure to a moderate range of cases in their specialty, while only 9 (20%) encountered a wide variety of cases. Conclusion: DRP was designed to strengthen the healthcare delivery system; however, this study reveals that the hospitals lacked basic infrastructure and had high patient loads. Residents reported moderate improvements in clinical skills, highlighting the need for better resources and support to meet DRP’s objectives.
