Oxidative Stress and Diabetic Neuropathy: Mechanism, Implication and Therapeutic Strategies

Diabetic neuropathy (DN) is a progressive and serious complication of diabetes that occurs at a strong prevalence rate that cannot be overlooked. For this particular review, our focus centers on how high amounts of glucose in the blood and the resultant oxidative stress influenced the development of DN. The authors highlight key processes involved in that degeneration including ROS production and the activation of polyol, hexosamine, AGE, and PKC biochemical pathways. These mechanisms lead to cellular damage, which ultimately affects the viability of neurons. Serving as a basis for action, the medical management of DN, or diabetic neuropathy, can primarily be dependent upon glycemic control. However, it is more effective in this regard for type 1 than type 2 diabetes as both have different underlying pathophysiology. There is a future present that looks towards controlling other factors alongside glycemic control such as oxidative stress which aims for even better management of diabetic neuropathy. Antidepressants, anticonvulsants, and opioids form a central part of pharmacotherapy for diabetic neuropathy, and it remains to be seen how new therapies will target the oxidative stress causing the disease.
