Spinal Dural Arterio-Venous Fistula: An Unusual Age of Presentation

Spinal dural arteriovenous fistulas (SDAVF) are the most common type of vascular malformation of the spinal cord typically found in elderly males. They are rare in children and difficult to diagnose. The clinical symptoms of spinal dural AV fistula are nonspecific progressive myelopathy, and the imaging features can mimic intramedullary neoplasm. SDAVFs should be considered when children exhibit acute neurological deterioration. Identifying the key imaging features is crucial in early diagnosis and appropriate treatment of this entity. Management of these fistulas depends on the findings of magnetic resonance imaging (MRI) and selective spinal digital subtraction angiography (DSA). It could be either treated by surgery or endovascular embolization with coils. We report a case of spinal dural arteriovenous fistula in a male child who presented with acute onset progressive back pain and gradually progressive weakness in both lower limbs
