OUTCOME OF PERCUTANEOUS NEPHROLITHOTOMY IN CHRONIC KIDNEY DISEASE PATIENTS - A PROSPECTIVE STUDY

Chronic kidney disease (CKD) is a common public health disorder that is defined as sustained kidney injury of more than 3 months resulting in a GFR of less than 60 mL/ min/1.73 m2 by Kidney Disease Outcomes Quality Initiative (K/DOQI) Advisory Board. it affects 229/million population in india. Patient with CKD represent 0.8%-17.5% of those presenting with urinary stone disease.The etiology of renal insufficiency in patients with nephrolithiasis is multifactorial and includes renal obstruction, recurrent urinary tract infections, frequent surgical interventions and coexisting medical disease. patients of ckd often have associated comorbidities like DM, hypertension.The likelihood of comorbid conditions may increase the operative risk. Percutaneous nephrolithotomy remains a standard procedure in management of complex renal stone disease.This study was done to evaluate the functional outcome of PCNL in CKD patients and to analyse the improvement in renal function after intervention. A total of 30 patients fulfilling the inclusion criteria were included in the study. Following the procedure 19 patients had improvement in renal function and 11 patients had deterioration in renal function signifying PCNL can be carried out with acceptable complication rates in patients with CKD.
