Abstract: Background: Sickle cell anaemia (SCA) is one of the commonest genetic abnormalities Worldwide, particularly in Sub-Saharan Africa, Nigeria bears the highest burden of this disease condition. Hypercoagulability in SCA may be responsible for the increased development of vascular occlusion in certain organs which may manifest as acute pain episodes. The causes of hypercoagulability in SCA are multifactorial and hypercoagulable state can be aggravated by high levels of Serpin A5, a protein C inhibitor. This study aimed to determine the association between Serpin A5 and bone pain vaso-occlusive crisis in SCA patients. Materials and Methods: Ninety-six participants were recruited into this cross sectional and prospective study done at the Lagos State University Teaching Hospital, Ikeja Lagos and comprised of 47 HbAA controls and 49 SCA patients. Five milliliter of venous blood was collected from each participant and 3 ml was used for the estimation of Serpin A5 levels of all study participants. The remaining 2 ml was used for hemoglobin electrophoresis and haemoglobin quantification of the HbAA controls and the SCA participants respectively. Data were analyzed with Statistical Package for Social Science (SPSS) version 23, p value was set at ≤0.05. Results: A total of 49 SCA subjects and 47 HbAA controls were recruited into the study. The various concentrations of Serpin A5 in SCA in crisis, steady-state and HbAA controls were 0.57 ± 0.14ng/ml, 0.51 ± 0.11ng/ml and 0.41 ± 0.04 ng/ml respectively. Comparison of the concentrations in crisis, steady-state, and control subjects was statistically significant with p=0.001 Conclusions: The mean concentration of Serpin A5 was highest during VOC, followed by steady-state and least in HbAA controls. There was a statistically significant difference between Serpin A5 levels in VOC and steady-state suggesting a possible association with VOC.
