Small jejunal diverticulitis is extremely uncommon, occurring in about 0.06% to 1.3% of people. Diagnosis is difficult since many people are asymptomatic or present with vague symptoms including malabsorption and stomach discomfort. Acute perforation may be seen in up to 6% of individuals. Here, we discuss a case of a 67-year-old man who underwent final surgery after presenting with vague pain in abdomen, multiple episodes of bilious vomiting. We emphasize the value of early resuscitation, a wide differential, and fast surgical therapy in complex jejunal diverticulitis. In this case for example, definitive surgery served as both a diagnostic and a therapeutic procedure, despite the fact that supplementary investigations like computed tomography may be beneficial in stable individuals.
