Abstract: A 24 year old pregnant (G2P1+0) female at 39 weeks of period of gestation with previous cesarean section two years back reported to hospital with pain abdomen. Since she had scar tenderness in previous scar site so she was immediately prepared for EmLSCS. She gave history of itching along the previous vertical scar line since two months, which later on led to extensive thick, rough hyper pigmented plaque with multiple punched out ulcerations involving abdomen extending 4 cm below the umbilicus and 3 cm above pubis symphysis, for which she was on medication. HPE finding from the lesion showed hyperplastic squamous epithelium with granulation tissue. She had diastasis recti with thinned out anterior abdominal wall. On first operative day, intestinal loops protruded through punched out ulcer as till now this anterior wall was being supported by gravid uterus. Conservative management for this patient was done and was referred to surgery department where repair of anterior wall was done.
