Abstract:
BACKGROUND There is growing evidence that pregnant women may be at increased risk of severe illness from COVID-19 compared with non-pregnant women.The spectrum of disease ranges from asymptomatic cases to severe and critical disease with pneumonia and adult respiratory distress syndrome being the most common serious manifestation, the others being thromboembolic and cardiac complications. METHODS.A retrospective case study of covid positive antenatal women who required inpatient admission was done and 7 critically ill pregnant women were selected and their course of events and management were revisited. RESULTS We describe the management of 7 critically ill covid positive pregnant women who were admitted in our hospital belonging to critically ill Covid. All seven of them required invasive ventilation and ICU care. Four women presented with desaturation at the time of admission. Six women had their pregnancy terminated during the time of admission for maternal indications. 5 women underwent preterm delivery. One woman was discharged undelivered after covid infection, but later went into preterm labour.One woman received ECMO; one woman needed prolonged ventilation and tracheostomy. One mother was readmitted on postnatal day 8 in view of worsening symptoms and she succumbed despite all efforts. CONCLUSION Our cases indicate that critically ill pregnant woman infected by SARS-CoV-2 may develop severe respiratory distress syndrome requiring prolonged intensive care, multidisciplinary team involvement and individualised approach of care ,including careful consideration of the expected risks and benefit of expectant management versus delivery and neonatal care in view of iatrogenic prematurity has to be sought.
