Thorner`s antagonism: A rare case of see-saw relationship between Myasthenia Gravis and Thyrotoxicosis

Abstract:
Thyrotoxicosis & myasthenia gravis (MG) are common diseases but their association is rare. Rennie (1908) was the first to report a case of exopthalmic goitre & myasthenia[1]. In 1939, Melvin W. Thorner reported seven cases of thyrotoxicosis & MG which exhibited a see-saw relationship i.e. mutual antagonism [2]. It was subsequently named Thorner’s antagonism [3]. In most other cases which were reported later, the patients died a myasthenic death following thiouracil therapy[4] or subtotal thyroidectomy[5]. A 38-year old male presented with symptoms of thyrotoxicosis and MG with the former being pre-dominant. After initial treatment with anti-thyroid drugs, the myasthenic component namely muscle weakness & ptosis worsened. He was started on oral Anti-cholinesterase agent, followed by a short course of Plasmapheresis. Thymectomy & total thyroidectomy were done subsequently.
